1) To be eligible to compete, the coach or team rep must submit the following items to North Country Region 3 WEEKS PRIOR to participating in the event. 

• $100  USD Foreign Athlete Insurance Fee/per team & Team Summary Form (Check made out to USA Volleyball): Mail to North Country Region, 4445 West 77th Street, Suite #237, Edina, MN 55435
  or
• Email Waiver for Foreign Athlete Insurance Fee & Team Summary Form (copies of written proof from each member of 24 Hour Health Coverage valid in the United States)
  
  and

2)  Coach or Team Rep must provide a copy of each item of all rostered members of the team to Tournament Director 3 WEEKS PRIOR to playing in the event.

• Government Issued Photo ID
• Letter of Good Standing from the team's National Volleyball Federation
• Completed Foreign Participant Registration Form
• Birth Certificate of each member under the age of 18
• USA Volleyball "Consent for Medical Treatment and Release" for each player

Once a Canadian Team registers for an event, notify North Country Region immediately

For a foreign team to be eligible to participate in an event, the coach or team rep must provide the tournament with copies of the following before tournament play begins:

• Government Issued Photo ID of all members of team
• Verification of Registration with the team's National Volleyball Federation
• Completed Foreign Participant Registration Form
• Birth Certificate of each member under the age of 18
• USA Volleyball "Consent for Medical Treatment and Release" form from all players
